NIH Employee DS-11 Official Passport or Renewal Instructions

This form must be presented and signed in person at the time of your appointment with the Fogarty Passport Agent.

You must use this form if:

  • You have never had a U.S. passport, or you need your personal passport for a personal trip and cannot submit it at this time, or
  • You are under age 16, or
  • If your most recent passport was lost or stolen. (If you are applying because your passport was lost or stolen, you will also need to submit Form DS-64.)

Requirements

When applying for an official passport, provide the Visa / Passport Office with the following:

  • Application for Passport (DS-11) completed except for signature. Please make an appointment with a Fogarty Passport Agent when using this form.
  • Do not write in the "Mailing Address" field (8). This portion is stamped by Fogarty.
  • Two identical photographs taken within the last six months. Photos must measure 2 by 2 inches on a light background. Pictures may be taken at the NIH Medical Arts Branch, Building 10 Room B2L328. Their phone number is 301-496-3221. They are open on Tuesday 9 am to 11 am, and on Thursday 3 pm to 4 pm. Please take an IC CAN number with you, which you can get from your Administrative Officer.
  • As of November 2016, eye glasses may not be worn in passport photos. If you cannot remove your glasses for medical reasons, you’ll need to obtain and submit a signed statement from a medical professional or practitioner with your passport application.
  • Original proof of citizenship must be submitted:
    • U.S. birth certificate with an embossed seal, or
    • Original naturalization certificate, or
    • Previous U.S. passport
  • If the name you now use is different from that on your birth certificate or naturalization papers, provide legal documentation (e.g., marriage license).
